WAITING LIST for Cardiac Screening for Club members (12-18yr olds) in partnership with Dylan Quirke Foundation

Unfortunately our first screening date has completed booked out.

However, we are really keen to raise awareness of SADS, facilitate the work of the Dylan Quirke Foundation and so try to offer this screening to as many members as possible. We are opening a waiting list for any cancellation that may arise for the 11th August AND to gauge interest so we can approach the provider to secure an additional screening date. Please complete the form to register interest in your child aged 12-18 being screened.

IMPORTANT NOTE:
1. If you have a pre-existing cardiac condition and/or are under medical care for this then this screening is not suitable.
2. If you are covered by Laya Health Insurance you can avail of cardiac screening through Laya every two years - please use Laya options for screening rather than using one of these screening places.

The screening is open to 12-18 year olds - there is a €20 fee for the screening which normally costs approx €100. We are able to offer a significantly reduced cost as the club is supporting this initiative through your recycling of bottles in our orange ReTurn bins - every plastic bottle/aluminium can returned will contribute to wellness initiatives throughout the club. Turn your bottles & cans with the ReTurn logo into a potentially life saving donation by dropping them into the bins at the club!


The Dylan Quirke Foundation relies completely on fundraising and we are so grateful to be accepted for one of their screening dates. You can provide a donation on the day of the clinic as they will have a 'Tap to Donate' facility at the clinic (no cash can be accepted).
